On page 1, section 4a, subsection (c), after the word, “immunizations” by striking the words, “set forth in subsections (a) and (b)”;

On page 1, section 4a, subsection (e), after the word, “service” by striking the words, “from the facilities listed in subection (a) and (b)”;

And,

Re-lettering the subsections accordingly.
